In that segment of the adult population at risk for hypertension and cardiac hypertrophy, hemodynamic pressure overload of the heart can lead to its enlargement and functional demise. Such enlargement results from an adaptive response on the part of the heart to alleviate pressure overload by providing increased contraction for more forceful pumping of blood. Cardiac hypertrophy also occurs during the normal development of the heart. As the developing cardiovascular system of the fetus places increased hemodynamic demands upon the fetal heart, the heart responds by undergoing hypertrophic growth to increase its size and contractility. Interestingly, many aspects of pathological adult cardiac hypertrophy appear to be similar to those of fetal cardiac hypertrophy, leading to the notion that adult heart cells respond to pressure overload by calling upon the fetal hypertrophic growth program to provide compensatory growth and increased contractility.
