TL;DRAbstract
Objective To describe a unique observation of nerve interpositions in the ulnar and median nerve, with a 40-year follow-up. Background Irreversible peripheral nerve damage is often treated with surgery. Nerve interposition is usually performed with auto grafts. Design/methods Case report, based on history, clinical details, NCV/EMG and imaging by nerve ultrasound and MRI. Case report A 60-year old patient had a complete lesion of the median and ulnar nerve following a plaster cast at the age of 10. The hand was severely impaired. Sural nerve grafts interpositions were implanted and with continuous training a substantial functionality of the hand was achieved. 40 years later the patient suffered from a mild loss of function and dexterity. NCV demonstrated prolonged distal latencies and small sensory action potentials at all fingers.
