Palliative Care Programme [poster]

TL;DRAbstract

One of the objectives of the Palliative Care Clinical Programme is to improve partnerships in care. A strengthened palliative care system is dependent on the interface and dynamics between the structural tiers in the system working collaboratively to deliver coordinated care. As such the Programme has set out to define admission criteria, transfer and discharge processes for all elements of the palliative care service by using standardised processes to ensure equity of access to, and optimal use of, services which will help to ensure that patients receive the level of care appropriate for their needs.
